On July 1, 2026, Medicare is launching a temporary program called the Medicare GLP-1 Bridge, which allows eligible Part D members to get certain weight management medications for a flat $50 a month. If you or someone you care for has looked into GLP-1 weight loss medications, you already know the sticker shock. Many of these drugs run well over $1,000 a month.
I know this bridge program will benefit many, so here is an overview of how the program works, what it covers, and who can use it.
The Basics of the Bridge Program
The GLP-1 Bridge is a short-term demonstration run by the Centers for Medicare and Medicaid Services (CMS). It runs from July 1, 2026 through December 31, 2027. The idea is to close a long-standing gap: Part D plans have generally excluded GLP-1 drugs prescribed purely for weight management, so this program “bridges” that gap until a longer-term approach is in place.
One thing to be clear about up front: this is not a new plan you sign up for, and it is not a benefit added to any insurance company’s plan. It is a Medicare program. You do not need to enroll, opt in, or take any action right now.
The Cost
Eligible members pay a flat $50 copay for a one-month supply. That $50 is the total out-of-pocket cost for the medication under this program.
A few cost details worth knowing:
- The $50 copay does not count toward your Part D deductible.
- It does not count toward your annual out-of-pocket maximum.
- Low-income subsidy help (Extra Help) does not lower the $50 under this program.
- The copay stays $50 no matter which phase of your Part D benefit you are in.
Which Medications Are Covered
Three medications are included when they are prescribed for weight management:
- Foundayo (orforglipron)
- Wegovy (semaglutide), in injection and tablet forms
- Zepbound KwikPen (tirzepatide)
Just as important, Ozempic and Mounjaro are not part of the Bridge Program. CMS may update the covered drug list during the program, but those two are not on it as of launch.
Who Qualifies
To use the Bridge Program, you need to be enrolled in Medicare Part D, either a standalone prescription drug plan or a Medicare Advantage plan that includes drug coverage. If you do not have Part D drug coverage, you cannot access the program.
Beyond that, eligibility is based on clinical criteria tied to your weight and related health conditions. The medication has to be prescribed for weight management, alongside ongoing diet and activity changes. Your doctor reviews your situation and confirms whether you meet the criteria when they submit the request, so the qualifying details are worked out between you and your provider.

This is the question that confuses people most, so here is the straight version. It depends on why you are taking the medication.
If you take a GLP-1 for a condition that Part D already covers, like type 2 diabetes, obstructive sleep apnea, or a liver condition called MASH, you stay on your regular Part D coverage for it. You would not move that prescription into the Bridge Program, and you would keep your normal Part D cost rather than the $50 price.
If you are already taking one of these medications for weight loss and paying out of pocket, you are not shut out. Once the program opens on July 1, 2026, you may be able to move to the $50 price, as long as you meet the criteria and your doctor submits the request. CMS has said this includes people who started treatment before the program launched.
There is nothing to do today. When the program opens on July 1, 2026, the process starts with your doctor. Your provider submits a prior authorization request along with a prescription for an eligible medication. CMS has set up a central processor to handle those approvals, the claims, and pharmacy payment. Prior authorization requests will not be accepted before July 1, so there is no rush to call anyone now.
If a GLP-1 might be right for you, the best first step is a conversation with your doctor about whether you meet the criteria and whether the medication fits your overall health picture.
